School Immunizations

Vaccines are our best protection against serious, sometimes deadly, diseases. To protect our children, Wisconsin law (5.252.04 Wis Stats) requires all students to do one of the following to attend school:
- Show proof they got required vaccines.
- Provide a waiver signed by a parent or guardian.

You can find and review your child’s immunization record on the Wisconsin Immunization Registry (WIR). The WIR is a secure data system that tracks immunizations given to people.  To obtain the dates of your child’s immunizations, type in your child’s name, social security or Medicaid number.
