- Blood Borne Pathogen Training
- Complete 
this form to acknowledge you have completed the required training.

Step 3: Complete the Substitute Information Form to assist in determining available placements.

Step 4: Make an appointment with HR Director, Samantha Hoyt, at 715-232-1642 ext.11062.

- Blood Borne Pathogen Training (you do not need to print out the results page)
- Complete this form to acknowledge you have completed the training.

Step 3: Complete the Substitute Information Form to assist in determining available placements.

 


Requirements prior to first day of employment

- TB skin test results (within the last 90 days)
- W-4 Form
- Direct Deposit Form
- Emergency Contact Form
- Blood Borne Pathogens Training
- Criminal Background Authorization Form
- I-9 Documentation Information: Passport or driver's license and social security card
