Step 2: View and Complete this online training presentation, watch the ALICE video, and then complete this form to acknowledge you have completed the required training.

Step 3: Make an appointment with HR Executive Assistant, Jamie Krista by calling 715-232-1642 ext. 11062 or using this online appointment calendar.

Step 4: Complete the Substitute Information Form to assist in determining available placements.

Step 1 :(Optional) Review the online training presentation.

Step 2: Watch the ALICE video and complete the Blood Borne Pathogen Training (you do not need to print out the results page) and complete this form to acknowledge you have completed the training.

Step 3: Complete the Substitute Information Form to assist in determining available placements.
