Download and save the packet to your device or print it. Complete pages 2-8 of the fillable packet, save again to your device, and submit these pages using one of the
following methods listed below.

Please note: The last two pages of the enrollment packet include Indicators of 4K Readiness and a Family Observation Checklist. This information is being included for families to help identify skills they can be working on in preparation for student success in a 4K classroom. This information does not need to be returned.
SUBMITTING YOUR 4K ENROLLMENT PAPERWORK

- Attach the required documentation as shown below to the email, or bring the documents to the district office located at 215 Pine Avenue E., Menomonie, WI

REQUIRED DOCUMENTATION
To complete the enrollment process, please submit the following items along with pages 2-8 of the 4K packet:
- Proof of Age: Child’s birth certificate
- Immunization Record: See page 7 of the packet and type in the dates in Step 2 or attached a printed record
- Proof of Address: A piece of mail or bill showing parent/guardian’s name and current address
- Additional Documents: (if applicable) Court or legal custody documentation
