Parents/guardians are requested to inform the school of any health problem and/or special health care procedures that need to be performed at school. This will assist the school in developing a plan to best help the student meet his/her needs.
If your child has any of the following:
Asthma
Diabetes
Heart Condition
Seizures
Severe Allergies
Or any other condition that may require an emergency care plan or procedure during school time.
Please contact the student health services coordinator to address planning needs. Health plans need to be completed annually.
HEALTH AND SCREENING PROGRAM
Screenings
Health Services provides vision and hearing screenings. Upcoming Kindergarten students are screened in the spring. Grades K, 1, 3, and 5 are screened in the fall with the help of parent volunteers. Please call your school if you would like to help. Screenings are also done at the request of a teacher or parent, etc. for all students throughout the school year
Immunizations
"IT'S THE LAW"
5.252.04 Wis. Stats.
This law requires students through grade 12 to be immunized according to their age/grade by the beginning of the school year.
Immunization is a shared responsibility:
- Parents are required to have their children immunized on schedule and keep accurate records. If a student is unable to receive immunizations for medical, religious, or personal conviction reasons, a waiver must be signed and given to the school
- Schools are required to keep records and review annually by the Immunization Law.
- When students do not meet the requirements of the law: Parents are notified by legal notice. If the student is not in compliance after receiving the legal notice, the district attorney is notified.
Helpful Hints:
*Keep a copy of your child's immunization record. Notify your child's school each time your child receives an immunization.
*Free immunizations are available through the Dunn County Public Health Department for people that meet eligibility requirements. The health department is located in the basement of the Dunn County Government Center
*There is a clinic the second and fourth Thursday of each month from 3:30 to 6:00 pm. A parent must accompany anyone under the age of 18 years. Call the Dunn County Public Health Department at 232-2388 to make an appointment or get more information.

+Varicella (Chicken pox) Immunization Requirement: A child meets the
requirement of the law if they had the chicken pox disease. Waivers are
available for medical, religious or personal conviction reasons.
~Kindergartners must have one dose of DTP (either the 3rd, 4
th, or 5th) after the 4th birthday.
>Kinderqarten and Grades 1-12 if your child received the 3rd dose
of Polio after the 4th birthday, further doses are not required.
-Grades 1-12 if your child received the 3rd dose of DTP after the 4th birthday, further doses are not required.
^Grades 6, 7, 8, 9, 10, 11, and 12: If your child received a dose of
tetanus or diphtheria containing vaccine such as Td within the past 5
years of entering the grade in which Tdap is required, Tdap is not
required.

Medication Policies and Procedures
Please give medications at home whenever possible. However, if medication is needed during the school day, the following is necessary.
Prescription Medication
- A doctor's signature with written instructions.
- Written consent from the parent for the school to give the medication.
- Medication in a properly labeled prescription bottle.
Non-Prescription Medication
- Written consent from the parent with instructions for the school to give the medication.
- Medication must be in the original container.
Medication forms are available at your school or local clinic.
- Students may not carry medication at school with the exception of prescription inhalers with a doctor's written permission.
- Please deliver medication to the health office.
- A new medication form is needed each time the medication is changed.
- A new medication form is needed for each school year.
School Illness and Injury Procedures
Students who become ill or injured at school will be seen in the health room or will be assisted by school staff where the injury occurred or where students became ill. If your child is unable to return to class, a parent/guardian will be contacted. For your child's comfort please make arrangements to pick them up as soon as possible.
